Under-eye bags are visible bulges beneath the lower eyelids. They are often caused not simply by fluid retention, but by the natural fatty tissue surrounding the eyes (orbital fat), which gradually protrudes forward. Their appearance varies considerably: some people develop under-eye bags at a young age, often due to genetic factors, while others notice them as part of the natural ageing process.

The eye is protected within the bony eye socket. Small compartments of fat provide cushioning. Towards the front, these fat pads are held in place by a thin connective tissue membrane known as the orbital septum. Over time, the septum and surrounding supporting structures lose firmness. As a result, the fat can protrude forward, a condition known as fat prolapse. At the same time, the lower eyelid skin becomes thinner and less elastic, while the orbicularis oculi muscle and underlying bone structures also change.
Beneath the fat pad, there is often a natural hollow known as the tear trough. Its supporting ligaments can pull the skin inward. The combination of protruding fat above and a depression below creates shadows that make the eye area appear tired.
Fluid retention, lack of sleep, allergies or certain medical conditions can temporarily increase swelling. However, in many cases, the main cause is protruding fatty tissue. In addition to surgery, innovative and less invasive treatment options are now available.
Brand new: BELLUTI Acoustic Shockwaves with the Diamond Particle Acoustic Patch.
This innovative non-invasive treatment combines a special black BELLUTI Diamond Particle Acoustic Patch with extremely short laser pulses.
The principle: Most of the laser energy is absorbed by the light-absorbing surface of the patch rather than directly heating the underlying tissue. This rapid energy absorption generates a photoacoustic impulse – a mechanical shockwave transmitted through the special patch and a contact medium into the tissue beneath.
These shockwaves are intended to act on the protruding fat and surrounding tissue structures. In simple terms, rather than using a hot laser beam to melt fat, mechanical impulses are intended to reduce the volume of the fat pads. Initial changes may be visible after the first session. The degree of fat reduction varies from person to person.

Special patches are placed on the skin beneath the eyes and treated using a specific laser protocol.
The procedure takes only a few minutes. The impulses may feel slightly uncomfortable but are generally described as well tolerated.
Most patients can return to their normal daily activities and work immediately afterwards. Temporary redness, small pinpoint bruises or swelling may occasionally occur. A completely downtime-free treatment cannot be guaranteed.
Depending on the initial condition, up to three sessions approximately three weeks apart may be necessary to achieve the desired result.
Improvements develop individually over several weeks. The method may be particularly suitable when under-eye bags are primarily caused by protruding fatty tissue.
This method involves inserting a very fine laser fibre beneath the skin through a tiny entry point. The laser energy acts directly within the tissue, using controlled heat to target fatty tissue while potentially stimulating additional skin tightening.
Compared with the BELLUTI patch treatment, Endolaser is a significantly more invasive procedure, although it can be performed relatively quickly.
The treatment is performed under local anaesthesia, with twilight sedation available upon request.
The procedure generally takes only a few minutes and does not require a visible incision or leave a visible scar.

Swelling and bruising may occur after Endolaser treatment, along with a feeling of tightness and temporary irregularities in the treated area.
Patience is important: tissue remodelling takes time, and the final result can often only be assessed after up to three months.
Occasionally, a second session may be beneficial. Suitability, potential benefits and risks always depend on the individual condition.
In cases of pronounced fat prolapse or significant excess skin, surgical lower eyelid correction (lower blepharoplasty) may be the more appropriate option.
Depending on the individual condition, the protruding fat is reduced or repositioned. If necessary, excess lower eyelid skin is also tightened.
Surgery provides direct anatomical correction but involves surgical and anaesthetic risks, swelling, bruising and a recovery period.
We do not perform this operation at our clinic. Where appropriate, we recommend consulting a specialist in eyelid surgery.
Our goal is to achieve a naturally refreshed appearance with a treatment tailored to your individual needs. We will be happy to advise you personally on the available options for treating under-eye bags at BEAUTYCLINIC.
Yes, depending on their cause and severity, under-eye bags can sometimes be treated without conventional lower eyelid surgery. At BEAUTYCLINIC, we offer two innovative methods: BELLUTI Acoustic Shockwaves with the Diamond Particle Acoustic Patch and minimally invasive Endolaser treatment. Both procedures aim to reduce protruding fatty tissue beneath the eyes. The most suitable treatment depends on your individual condition.
Under-eye bags often develop when the natural fatty tissue surrounding the eyes begins to protrude forward. Genetic predisposition, ageing and weakening of the supporting tissue structures can contribute to their appearance. Fluid retention, allergies and lack of sleep may also temporarily increase swelling.
BELLUTI Acoustic Shockwaves is an innovative non-invasive treatment developed in Korea. A special Diamond Particle Acoustic Patch is placed on the skin beneath the eyes and treated with short laser pulses. The resulting photoacoustic shockwaves are intended to act on the underlying fatty tissue and reduce its volume. The procedure takes only a few minutes and generally requires no anaesthesia.
BELLUTI is a non-invasive treatment performed using a special patch placed on the skin. Endolaser, on the other hand, involves inserting a very fine laser fibre directly into the tissue through a tiny entry point. The laser energy generates controlled heat to target fat and stimulate additional skin tightening. Endolaser is more intensive but also more invasive than the BELLUTI patch treatment.
With BELLUTI Acoustic Shockwaves, up to three sessions approximately three weeks apart may be recommended, depending on the severity of the under-eye bags. With Endolaser, one treatment is often sufficient, although a second session may occasionally be beneficial. The number of treatments depends on the individual condition and desired results.
With BELLUTI, initial changes may be visible after the first session, with further improvements developing over several weeks. Following Endolaser treatment, the tissue requires more time to recover and tighten. Final results can often be assessed after approximately three months.
BELLUTI treatment may cause a mild tingling, tapping or warming sensation. Anaesthesia is generally not required. Endolaser is performed under local anaesthesia, with twilight sedation available upon request. Some pressure or pulling sensations may be experienced during the procedure.
Most patients can return to their usual activities immediately after BELLUTI treatment. Temporary redness, mild swelling or small bruises may occasionally occur. Following Endolaser, swelling and bruising are more common, and a few days of recovery may be advisable depending on the individual healing process.
Both treatments aim to achieve a long-lasting reduction in protruding fat. However, the durability of the results varies between individuals. As the natural ageing process continues, additional fatty tissue may gradually move forward from the eye socket. Permanent and complete removal of under-eye bags therefore cannot be guaranteed.
The most suitable treatment depends on the cause of your under-eye bags, the amount of protruding fat, skin quality and your personal expectations. BELLUTI may be an option for mild to moderate fat protrusions, while Endolaser may be considered for more pronounced fat deposits. In cases of significant excess skin or severe fat prolapse, surgical lower eyelid correction may be more appropriate. At BEAUTYCLINIC, we assess your individual needs and discuss the available treatment options.
Treatment costs depend on the chosen method and your individual condition. We will be happy to advise you personally at BEAUTYCLINIC.
