Dark circles under the eyes are caused by a visible depression under the eyes through which the veins shimmer, making the skin appear bluish. In addition, the skin can also be darkly pigmented; mostly constitutional, but often also due to iron deposits in the skin.
Dark circles can be inherited, the eye area is structured in such a way that the edge between the lower eyelid and the face is not filled and therefore creates a shadow. There are other causes that can lead to dark circles such as tiredness, stress, poor diet (e.g., iron deficiency symptoms appear in the eyes) and the consumption of harmful substances such as alcohol and nicotine. But very often the ageing process is a common cause of dark circles. Circles under the eyes are caused by two mechanisms as part of the ageing process:
Covering up extreme dark circles is not easy and the dark shadows are usually perceived as disturbing. If the situation cannot be improved by lifestyle changes, cosmetic treatment of dark circles may be considered. For optimal treatment, it is very important to analyze the cause of dark circles under the eyes, as this is the only way to create an individual treatment plan that will help to remove them. Due to the many different causes of dark circles, ten patients may receive ten different treatment suggestions.
The treatment of dark circles is based on two strategies: Firstly, the "groove" under the eye is filled in, and secondly, the color is treated. Dark circles can be treated without surgery. However, depending on the findings, several treatments may be required to achieve a satisfactory, fresher appearance.
To fill the canal, the patient 's own fat should primarily be used, which is injected here as nanofat. The fat is obtained by means of a small liposuction using the Tulip® Medical System, currently the best nanofat treatment method on the market. Fat is yellow and is therefore ideal for lightening the skin and never leads to swelling or dark discoloration, which can be possible with a dark circles treatment with hyaluronic acid. Although transparent, hyaluronic acid often appears dark blue to black under the eye. Due to its capacity to absorb water, hyaluronic acid can also often result in unsightly puffiness. Hyaluronic acid is a wonderful filler for filling in sunken areas of the face, which is why we use hyaluronic acid exclusively to reconstruct lost fat in the midface. Autologous fat is ideal for treating dark circles because it does not lead to discoloration and prevents the formation of nodules. The results of a dark circles treatment with autologous fat last for five years or even longer.

Lightening the dark color pigments of the skin under the eye is generally very difficult and usually requires several treatments. The cause of the discoloration is often an iron pigment that arises from the venous plexuses under the skin due to leaked mini blood droplets. Sometimes, however, it is also due to recoloring pigment cells.
Discoloration of the skin is generally difficult; it often takes several treatments with different methods (e.g., platelet-rich plasma, fractionated CO2 laser, serums with vitamin C, carboxytherapy, etc.) until the result is satisfactory.
Dark circles under the eyes can be treated with PRP, platelet-rich plasma. These are the body's own components from the blood which, once injected, burst open and release helpful hormones, enzymes and vitamins that can lighten the skin and also improve the volume. However, it takes several treatments, usually 3-5 sessions, to achieve a good result. PRP is usually combined with an autologous fat treatment.

In carboxytherapy, carbon dioxide gas is injected under the skin, releasing the free oxygen in the tissue and thus lightening the discoloration through regenerative processes. Carboxytherapy is the current megatrend from South America (Brazil). There are no side effects. The treatment does not take long and is painless. It takes many treatments before results are seen.

For the treatment of dark circles under the eyes, the laser is used fractionally, which leads to small "holes" in the skin, over which new skin cells grow, which lead to the "discoloration" of the dark circles. As an alternative to the fractional CO2 laser, we use the Nd:YAG laser, which treats the dark color particularly well. Immediately after the treatment, the facial skin is slightly reddened, but you can go straight back to work. Both laser treatments must be repeated until good results are visible.

Fat transfer is considered the most stable filler for the under-eye area. Once integrated, the fat stays permanently in place and doesn’t migrate. The only limitation is that the body may not absorb all the fat evenly, so a second session is sometimes needed. The fat that successfully integrates remains for life.
