In the past, implants filled with table salt or soybean oil were used – these are no longer used today. The filling material is decisive for the softness and shape of the breast to be augmented. In earlier years, liquid silicone was used; today surgeons use a cohesive, cut-resistant silicone gel that can no longer leak.
Depending on the consistency of the mammary gland tissue, a choice can be made between round and so-called anatomical implants, which have a teardrop shape. Furthermore, the exact implant height, thickness and width must be adjusted so that your breasts look as natural as possible. Information about the manufacture, quality, durability/guarantee and associated costs of the breast implants available on the market is also very important. The aim of the desired breast augmentation should be to fill out the existing breast tissue without parts of an implant becoming visible.

Breast augmentation surgery should always be performed in a hospital or specialist clinic by an experienced surgeon. Dr. Gallenkämper performs all procedures himself and the operations take place in various affiliated hospitals in Zurich and central Switzerland. The operation itself is usually performed under general anesthesia and takes 1-1.5 hours. During the operation, the patient is sat up so that the position and projection of the new breast can be assessed and corrected if necessary. In order to be able to react flexibly, the surgeon always has several implant sizes to hand during the procedure.
Breast augmentation is usually started via an "incision" underneath the breast. The incision is about 5 cm in the underbust crease, which is the safest and least visible method (alternatively: armpit or nipple incision). Once the implant has been inserted, the skin is closed with a suture; immediately after the operation, a firm breast wrap is applied, which should be left in place for 24 hours. Post-operative pain is treated with mild painkillers, the first dressing change with removal of the inserted suction drains usually takes place after 24 hours. During the dressing change, you will be fitted with a so-called compression bra, which ensures the exact positioning of the breast implants and should be worn day and night for 4-6 weeks. The follow-up checks are very important: These should be carried out between the 1st and 2nd week and after the 4th week, then between the 3rd and 6th month, and annually thereafter.

We differentiate between anatomical (teardrop-shaped) and round implants. At our clinic, we exclusively use implants with a textured surface – similar to velvet or suede. This structure ensures that the implant remains securely in place within the breast pocket and does not move. Additionally, the textured surface significantly reduces the risk of capsular contracture compared to smooth-surfaced implants, which are now rarely used.
Anatomical implants offer a much more natural look than round ones, as they mimic the natural shape of the breast.
During your consultation, you will be able to view and feel various implants and share your preferences regarding future breast size. In principle, you can choose the size you want, provided your tissue allows it. Extremely large implants may require pre-expansion with so-called expanders. As with any surgery, there are three basic risks: infection, bleeding, and scarring. Thanks to our sterile surgical procedures, the infection rate is below 1%. To further reduce the risk, you’ll receive a one-time short-acting antibiotic during the procedure.
Bleeding, if it occurs, usually happens within the first 24 hours. Often, a hematoma can be controlled with compression, but in rare cases, surgical removal may be necessary.
Even the best surgeon cannot fully prevent visible scarring. However, if scarring occurs, it can be corrected later under local anaesthesia.
The main risk of breast augmentation is capsular contracture – excessive internal scar formation around the implant. Every foreign object – whether a pacemaker, joint implant, or breast implant – will be encased in scar tissue by the body. Normally, this capsule remains soft and unnoticeable. In rare cases, if the body overreacts (often only on one side), the breast can feel hard. If there’s no pain or aesthetic distortion, treatment is unnecessary. But if the breast becomes painful or visibly deformed, the implant must be replaced.
With the latest-generation implants from Polytech Silimed that we use, the risk of capsular contracture is under 5% (compared to over 30% with older smooth implants).
